At our school, we are happy to administer both prescription and over-the-counter medicines to pupils when needed. To ensure this is done safely, a completed medication consent form must be provided before any medicine can be given. Forms should be filled in by a parent or carer and handed directly to the school office. You can download the form by clicking on the file below.
